This Legal Contract for Name Change (“Contract”) is entered into as of the date of execution, by and between the undersigned party (“Petitioner”) and the District of Columbia Courts, in accordance with the laws and regulations governing name changes in the District of Columbia.

WHEREAS, the Petitioner desires to legally change their name from [current name] to [desired new name], and the District of Columbia Courts is responsible for overseeing the legal process for name changes;

NOW, THEREFORE, in consideration of the mutual covenants and agreements contained herein and for other good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, the parties agree as follows:
